This web page provides currently available template versions of contracts, called Master Service Agreements (MSAs), for these Qwest Wholesale Commercial Solutions products as well as copies of customer specific, executed MSAs. Qwest invites you to review the MSA templates below to determine if you wish to enter into a commercial business relationship with Qwest.

To request preparation of an MSA, contract your Qwest Sales Executive. Qwest will create and distribute to you two original MSAs with a customer specific tracking number. Sign both documents and return them to Qwest for signature. Qwest will retain one original copy at our Contract Development & Services location as well as post a copy to this web page. Qwest will return the second signed original to you by mail.
